Hi JotForm Support Team:
I reached out a while back and sent a couple follow up emails with no response. I'm assuming my ticket was closed (my email requests weren't showing up on the original thread) hence the lack of response, but I still need an answer to the following:
In order to process a check, our school district has required that we receive a vendor application from JotForm (blank form attached), and a W9 for both JotForm and BlueSnap (linked here). Are you able to send these my way? If not, can someone from your team send a message back with further context?
I can't seem to attach the vendor application here because the form will not allow that file type. Please advise as to how I can send.

I see that the thread has already been addressed by one of our support team members. Since our subscriptions with offline payment methods are managed by BlueSnap, the W9 from BlueSnap should be sufficient. Here is the link to the W9 form for BlueSnap.
