Exam forms form Ponsonby School of Dance, unable to use?

Hi, we have a link provided by Ponsonby School of Dance. This link is for an exam form.

When we click link, we are taken through to web page with a message saying, This form exceeds quota.

Could would please help us work out how to find this form.
