What's the easiest way to go about making my form?

    At our church, we are beginning registration for classes in a couple of months.  The form must be a card because there are specific questions we need to ask for certain answers, but I want to get feedback on the easiest way to go about making this form because it is extensive.  

    We are registering for four different types of classes (Preschool, Elementary, Middle School, and High School--all of these during different times/days of the week) and there will be multiple children registered per form.  The form will be filled out per family.  

    So the idea is that the family can select the number of children they are registering, select which type of classes each individual is taking (pre-k, elementary, middle school, high school), and then fill out the information per child they are registering depending on what class they are taking.  Each class will have different questions they will need to fill out (for example preschool will need to indicate if child is potty trained, but this will not need to be asked for middle school/high school).

    What is the best way to go about setting up the form so that there does not have to be a great amount of conditions? I would rather not have so many conditions if not necessary.  Any suggestions on the best way to go about setting this up? Thanks!

    I'm afraid that you will have to go with the conditions.

    Since you have a question asking your users how many children they want to sign, then according to the number of children selected you would show the classes to be entered for each child, as well as the information related to each child.

    So you could use a dropdown, for example, to ask for a number of children:


    And show (conditionally), based on number chosen, the fields for entering the rest of the data.
