I can't get the form to pop up on my Shopify-powered website

I cannot get the form I created to pop up on my shopify store.  It’s a bar at the top and then opens in a separate window.

Is your goal to make it appear as a lightbox popup, where it shows straight inside your website and not in a separate window? If so, then I recommend you remove the pop-up embed codes from that page and use the LIGHTBOX EMBED codes this time.
