I want the person filling out the form to choose who the results go to. Is that possible?

    Hello there!


    I would like to build a get to know you form.  For example, I would give you this form saying:  "Hey, I'd like to get to know you, will you fill this out?"  Ideally, at the end of the form there would be two spots for them to choose where the results go.  The idea is that they would send it to the friend asking them to fill it out and that they could receive the results as well and that it would NOT go to JotForm.  Is this possible?

    I have created a demo form as per your requirement. I have added 2 email fields. If the user selects to send the form to himself, the other will be hidden. If a user chooses that he/she wants to send the filled form to a friend, another field will come up which will ask them to add their friend's email address.

    I have also included the link of this form in the notification which will be sent to the friend. If he/she will click on that link, they will be direct to this same form.
