Duplicate input fields

Hi, I need to create a form where people can enter details for a number of items.

e.g.:

.........................................................

Date:

Reference:

Details:

CSR:

_________________________________

Insert another request (as a button)

....................................................................

If a user needs to insert another request the section of the form I described will copy itself and the user can fill in another request.

Can anyone provide direction or point me at a template that I can copy, so far I have not found anything useful in my searches.

You may like to take a look at our "Configurable List" widget and see if that fits into your requirement. This widget will allow your users to duplicate the same form fields multiple times. There is also another widget that might work as well, ADD Options widget: https://widgets.jotform.com/widget/add_options.
