How to create a Form with Yes/No questions in it?

    Hey JotForm Team,

    I'm working on creating a JotForm survey that requires my company to receive a yes or no response from the individuals the form is being sent to. 

    The way we are formatting the survey - if they indicate "no" on a certain question on a specific page, we would like the survey to conclude then and direct them to a thank you page for their submission. However, if they were to indicate "yes" we want the survey to continue on to the other pages we have built. 

    Is there anyway that I can create the survey so that this takes place? 

    Any help would be greatly appreciated. If this does not make sense, I can be reached by phone at 424-245-3693 until 5:00PST as well as tomorrow, Thursday from 9-5 PST

    You can create a Cards form, add Yes/No question to your Form. Next, add a condition to your Form that if the user has clicked on "No" then it should skip to the last page where you have added a Text element to show a custom Thankyou message.
