The PDF output of the form is missing fields, and fields are out of order

    I have a web based form and would like to receive the submission in PDF format. When I receive the PDF, it's missing a number of fields that are in the web form and the ones that do appear are completely out of order. 

    It's possible that I may have inadvertently messed this up at some point, so is there a way to reset the system to automatically generate a PDF of exactly what's on the web form?

    You can re-generate the PDF document by going to https://www.jotform.com/myforms, click More - Create PDF Document:

    When in PDF editor, you can create a new PDF at the upper left corner, by clicking New PDF:

    That will create a new PDF with default layout (all fields supported will be included).
