"sorry Multiple entries are not allowed"

    Hello, I wanted to see if this form was working because I am not getting any emails stating that people are filling out the form so I did a test one and it says "sorry Multiple entries are not allowed"  Please let me know what this means and how to fix, thank you, my phone number is 561-488-8826 ext 135.  Please ask receptionist to page me if i don't answer it's urgent.Thank you, Jaime

    We apologize for any inconvenience this may have caused. The form you are currently using Unique Submission is currently set to not allow multiple entries. To allow user to submit multiple entries in the same computer/email, please go to preference and set Unique Submission to No Check
