Why is my account suspended

    Hi, I am wonder why after 2 years my account has been suspended.


    I thought there was just an issue with log in, so I changed my pw and 

    it is still telling me that my account is suspended? Why?

    Upon review of your account you are requesting with your form the collection of government issued IDs and also a digital copy of them which is not allowed since they are considered sensitive informtation.

    We will only be able to reactivate your account if you allow us to delete the guilty fields or buttons.

    We will wait for your response.

    Unfortunately I can't delete the form because the FDA is requiring us to
    get pictures of peoples ID as proof they are old enough with a Valid ID.
    Sorry , but this is FDA policy and I will find another way to get our
    customers out of town DL so that we can have it on file with the FDA

    After taking a closer look to your webiste, and since you are using the forms for a vape company, you are required by law to request for IDs, with this in mind your account has been reactivated, kindly add this missing widget:

    Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    Your account has been added to our whitelisted database in order to avoid future inconveniences.

    In all honesty this is very important, and I actually had a customer on
    the phone and had to make a new account. So we have a new account
    TexasIDsubmissions and I did go in and edit it with a widget. If I need
    anything else on that please let me know. Or maybe how to word it.

    As long as you have both widgets and your corporate email address, that allows you to request ID upload due to the nature of your company.
