Registry Widget being cut off

Hi there - 

I added the registry widget to my form but the age groupings is getting cut off. I've tried modifying the width and height, but it makes the whole table disappear. 

Please help! What am I doing wrong?

We would like to apologize for any inconvenience. I have checked the form you linked on a desktop using Google Chrome as my browser and I was not able to reproduce the issue you have described.

We would like to help you with your concern. Would you be able to tell us how we can replicate the issue, please? May we know what device and/or browser where you using when you access the form? Also, is the form embedded on a website and the issue is only happening there?
