How to remove the broken graphic link at the top left corner on my form?

Also there is a broken graphic link at the top left hand corner of the form when it begins. How can that be made to go away?

It shows up in both versions for me.

The tooltip says that "Only Form Owner sees this button". But since I'm looking at an embedded version of the form, and the button image doesn't appear, I'm wondering what else is going on.

I believe the image you are referring to might be associated with smart embed:

Try viewing the page in private/incognito mode or when not logged in to your JotForm account.  The image should only be shown when the form is viewed by the form owner.
