Our FREE account is suspended. Why?

    Our entry level account (FREE) is suspended and we can't sign in or access the forms we created? Why? We received no email notifications about the suspension, or what to do to get our account reactivated. Need help regaining access, please. Thank you.

    This form http://www.jotform.us/form/82835161539159 was found on your account as suspicious of phishing activity because you're collecting driver's license upload.


    Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    You may collect driver's license numbers if your purpose is not to collect these data from the general public, but from your own customers, you just need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, social security cards, driver's licenses, credit/debit cards and statements, bank statements, voided cheques/checks, birth/marriage certificates, visas and photo IDs.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.

    It would have been nice if you'd advised us of this violation, before
    simply suspending our account and deactivating our Employment Application
    form without notice. We require Driver's License information as part of our
    driver's PUC licensing to be off-road jeep tour guides. This dead (now
    deleted) Jotform was on our website, which makes us look unprofessional to
    potential job applicants.
    I am forwarding your email to our Fleet Manager, so he can make the
    required changes to the form. Thank you.
