Missing form seems to have merged with another form

    On March 25 I created a form named "Emergency Contact Info". This form is now missing from our folder, and its original URL now directs to https://form.jotform.com/90836919323161. Is there a way to recover the original form and figure out how/why responses to the original form are now being populated into the "Travel and Education Request" form?

    I checked your tjacobson account and I can not see any form creation named Emergency Contact Information. The only form with "Emergency" form name is this: https://support.jotform.com/form/60826640460958

    Could you please give us the form URL of the "Emergency Contact Info" form you are referring to so we can trace or check it?

    The original URL was https://form.jotform.com/90836919323161; however, this no longer goes to the form.

    Form ID is unique and it is not possible to merge two forms. Unless you make changes to the form and make it look like the form now.

    I also see many changes made in the form, you can view it in your revision history: How-to-View-Form-Revision-History

    Perhaps the owner of the form made changed to the form you mentioned? Or that account is also yours?

    Yes, there were a number of changes made; however, on April 17 there is only 1 change listed but on that date the form went from emergency contact to travel and education request. How could that be?

    I think I fixed it. I cloned the form, then reverted the original to a previous state.
