HIPAA Form Status: it is sometimes not available, it says "disabled by owner"

    I have created a form for NWCT and sporadically I can view form and submit information to it and have it email me response. Other times, it says form was disabled by owner. That would not be possible as I am the owner.

    Could you please check if the form is set up to be disabled?

    If the issue happens again, please let us know when(date and time) exactly you saw the error message.
