Customization/Formatting of Forms

    Hi,

    I'm looking at purchasing jotform for our veterinary hospital use - for new client registration forms, surgical release forms, wellness questionnaire forms, etc.

    However, after playing with the form creation, I've noticed that the output of the forms takes up several pages. I need to consolidate the information as much as possible, without cutting down on the questions we're asking. Currently, we use a written/printed pdf and are able to fit all of the information onto one sheet.

    With jotforms, is there a way to customize/consolidate this information into one sheet that I'm just not seeing? I'm not a programmer. Alternatively, if there is a way and it's above my paygrade, is there an option to have someone else create the forms for us?

    I have checked your form and it seems to have a number of question. Do note that when you output your submission as PDF it has its limit in height and width(A4,A2,A1).

    You may edit your PDF document in your PDF editor.

    Go to your submissions page and click on PDF>Customize PDF

    In your PDF editor you may delete some fields by selecting a field and clicking on the trashbin icon.

    You may also move your fields and adjust them.
