thank you

Hi @fjfedato!
You can simply add a Short Text Entry element under the single choice question, like so:
I have created a sample form for you, which you can clone and use if it meets your requirements!

Thank you, very much. But I still have questions:
How can I add a field for comments in the SAME single choice question (like you did)? When I add a short text, this command is creating a new question.
Can I clone just a question on a existing form that I'm editing? (Not clone a entire form when I want to create a new form).
Thank you

Yes, when you add a Short text entry as suggested by my colleague, it would add a new field.
If you want to show the comments only when the single choice would be selected then you can write a condition to your Form that when the single choice is selected then only show the comments section for the single choice else hide it.

Thanks, but my question is different (and easier than show/hide). How can I add a field for comments in the SAME single choice question (like you did)? When I add a short text, this command creates a new question (not only a new field), like this:
and I would like the field comments in the same question, like this:
(not a 5th question with only the field comments)
thank you very much

As you are trying to add two fields together in the Cards Form? Unfortunately, it is not possible in the Cards Form to add two fields in a single Card.
Card Form has a special field called "Multiline element" using this you can add multiple elements in a single card but this does not support Single choice option.
It supports the following elements, you can add any of these elements multiple times in a single card:
In Classic layout Form, it is supported to add two fields in a single page. If you want then you can switch to All questions to one page:
Let us know if you have further questions.
Thanks.

To be able to create above you need to create a Form using the Classic Layout. Please follow the screencast below to choose the Classic layout.
After you choose the current layout then you can add Single choice option as well as the comments section.
Since you are having a Cards Layout in this Form so you won't be able to do it here. You can change the layout of your existing Form from the Form settings:
Thanks.
