How to create a pedigree chart like form?

I would like to create a fillable pedigree chart  like this for one of our applications.  Is there a widget that is already designed to do this?  Is there another way.  I am not able to write code

You can try using Jotform's dynamic dropdown widget https://widgets.jotform.com/widget/dynamic_dropdowns. It's not exactly a chart like widget, but you can add sub-options based on the first selection.

That looks like a great option for some things, but I need something like an excel sheet wherein only certain cells are fillable.  Would be great if I could provide hint text in those cells.   

The only widget that resembles an excel sheet is the Spreadsheet widget.

You can configure the spreadsheet widget to have rows/columns that look like the pedigree chart that you have shown us.

Could I hire someone through you to create the proper CSS.

I would need some cells read only and other fillable.
