Group Membership Form

I need to be able to add up to 50 memberships to my form and submit one payment. 

For example, if you answer yes to "do you want to add another member" I need it to return to page 2, clear the previous data entered and add another member, keeping all of the conditions for page 2.  Once you are finished adding members by answering "no" then it takes you to the payment page to pay for all of the memberships.
