completed forms are no longer being sent to my registered email address

    You have a condition that controls sending of the Notifier:


    That requires the name field to be filled in order to send Notifier. And the name field is not required field. Meaning, when the name field is not filled, the notifier will not be sent.

    You can disable or delete that condition and test and see how it works.
