Can we add fill-able questions within a text entry?

Hello,

We want to use Jotform for our Speaker Agreement. We want to ask them the title of their presentation and when they will present. Is there a way to add fill-able questions they can answer within a text entry? We also want their name to be on the letter, is there a way we can add a name text for each person for each agreement?
