Submissions Problems? Not recieving them, thanks

I cloned your form 12140320181 and performed a test submission. But, the form submitted successfully. Are you referring to the error below?
I see that Unique Submission field is set "Check cookies only". To resolve the issue, please navigate to Settings > Form Settings > Show More Submissions then scroll down to Unique Submission and set the field to No Check.
