How to prepopulate form values from another form?

hallo,
we have a hairdresser salon  and we would realize this :

A customer fill a form ( https://form.jotformeu.com/91083805196360)

He gives us in fact 2 possible dates to visit our business (unique fields : datum 1 and datum 2)

When he submit the form:
- we receive a notification (notification 1) with his submission information
- He receive a mail to notificate him , we will contact him back later with an appointment purposal (autoresponder 1)

Our issue 

We want to do is :
reply him from the submissions page  with a link to a 2d form (https://form.jotformeu.com/91133625758360) so he can confim us an appointment by submitting the form.

we want to prepolutate some fields of form 2 with answer coming out form 1

unique field mev7 (form 2) with unique field FullName22 (form1)
unique field typebezoek (form2) with unique field voorwelke (form1)
unique field trackid (form 2) wirh unique field typeen (form 1)

we have readed different topics in the forum but we don't find the right answers to realize this.

Is it possible to realise this ? could you help us ?

Please note that it is possible to pass the field values from one form to another by following the guide below:

However, please note that the link should be generated only when the first form is submitted. The generated link can be copied to Thank you page displayed on the first form or the email autoresponder sent to the customer.

is it possible to disable the form for one customer but to use this same form for other customers and so on ... ?
