Question label is not being displayed in submission emails.

Thank you for your great website... it's very helpful.

When I get the email from responses from people.  I'm not getting the questions on all the questions see attachment.  Questions 3-5 are missing in the left column... the right column says "nope".

I am sorry for the trouble caused to you. I did check your form and found that the issue is caused because you have not added question label but just added description. Please check the screenshot below:

Please note that question description is not displayed in submission email. I would suggest you to please edit your notification email template and manually add the question label. Please check the screenshot below on how to edit email alter template:
