Need help refining my payment form

I need to create a limo submission form with credit card collection as well I have created something already but it need refining please.

You never specified what kinds of changes you need though. You just said that you need to further refine it.

Looking at your form, I saw that you used the AUTHORIZE.NET payment field. However, it wasn't fully configured yet. I strongly suggest you set up your API Login ID and Transaction Key inside the payment field. If you don't know where/how to get these info, you may refer to this help page from authorize.net:
