    I think I should start by explaining how online web forms work in general. The form you built above doesn't have any input fields. What this means is that it won't ask for any input from your users. Your form only has a text field that displays a couple of paragraphs and it doesn't have a submit button.

    Now, I noticed you also built a fillable pdf form. Since this pdf form will just follow whatever fields you have in the web form, this too won't be submittable.

    If your goal is to create a web form, please use the FORM BUILDER and make sure you will add regular form fields (or input fields) to your form. Otherwise, if you wanted to build a fillable PDF form (and not an online web form), use our PDF EDITOR instead and make sure you will add form fields and a submit button as well.
