Why is my form not updating?

Hi Guys - hoping you can help. For some reason I can't get my form to update on the front page. I've added on a logo and changed the title heading which is showing in the design environment, but as soon as I preview the form it doesn't show. Not sure where I'm going wrong so if you can point me in the right direction that would be great. Thanks Kerrie

I cleared your form cache and this seems to have fixed it. Can you check your form again?
