An in office use only area that is hidden from the submiter

Hello, 

Is there a way to add to each of my forms 

Appointment Scheduled? Yes/NO

Notes: ______________(text field)

But I what this hidden from the submitter and the office here would only fill this out after the form was submitted.

You may add the fields to the form and hide them from the field properties. When the submission is edited by logging into your JotForm account, the hidden fields should be displayed so that you should be able to add the information to the fields. 

If you want to display these fields by using some sort of code, you may hide them by using a condition

When the submission is being edited, the hidden fields can be shown by entering the code to make the changes.
