Your forms and submissions are available in your account. The reason why you are seeing otherwise is that your account is suspended due to the "Hostess Order Form". Let me forward this to our review team who will respond to you accordingly.

The forms are not linked to a corporative email address. We understand independent contractors work this way. Nevertheless, we need a one time requirement of such proof in order to whitelist your account.
