Request to transfer a form?

Hello, I have officially started a second business and need to move forms I created under my old business's account to a new account.  Can you help me with this?

The forms are under the username catchaspark.  Specifically I want to move two forms and their submission data:  "CYS - Adult Model Release" and "Personal Brand Photography Planning Questionnaire".

The new account is under the username catchyourspark.
