My Square widget is not rendering on our form

My Square payment widget is no longer showing on Preview or the Published URL.  It displays on the Edit screen just fine.  I have a lot of configuration time invested in this widget configuration so please let me know what needs to be changed instead of changing it for me.  Thank you.

It seems that there was an issue with saving the form. I have tried moving the email address field below the Submit button and move it back to above it. The form is now displaying the fields without any issue. Please check and let us know if you need any further assistance. We will be happy to assist.
