How do i create a jot form?

Once you log in to your JotForm account, you will be taken to your My Form page. Here you can select "Create Form". After that, you will need to confirm the Form layout - Classic or Card form.

Classic forms have all questions in one page, while for Card forms, there will be one question per page. Once the form layout is confirmed, you can check the screencast below:
