Form loading wrong pdf and wrong questions

    I complete the 30 Day Check-in JotForm.  It emails it.  It attaches a pdf version with the correct answers but when you open it, the questions are the wrong questions and the title is Employ exit surveys which is wrong.  How are these getting mixed up in such a complicated way?  Wrong title, wrong questions but correct answers.  It shows fine in the email until you open the pdf document.

    This might be caused by an outdated PDF document template.

    If you provide us with a form link, we can take a closer look.

    The PDF created from an entry coming from the following link, it’s clearly crossing the forms data when creating the PDF. It does not appear to effect the excel downloaded version, just the PDF.

    We apologize for any inconvenience caused. What we're suggesting is to reset the PDF document being used by the form.

    1. Load the form in your PDF editor

    2. In the PDF editor, click on +New PDF tab and select New PDF Document option

    3. Select a layout and then click on Create button

    4. Save the PDF document

    5. Go back to the Form Builder

    6. In the Emails / Advanced tab / PDF Attachment option, click on the dropdown to select the name of the previously saved PDF document from the PDF editor.

    This will make the form used the updated PDF document/report.
