No notification is received when someone submits my form

I created a form yesterday and sent it to people. One told me she responded, but I never received an email informing me, and I don’t even see any created forms on my account. Here is the link to the form I created:

I checked the submissions page of the form you sent us and I see two submissions. The reason why you did not receive an email when someone submits your form is that you do not have a notification in your form.

Please add one so you will receive a notification when someone fills and submits your form.
