One of my forms is not working correctly

One of my forms is going to a white page near the end of booking before payment and I have looked through everything I can think and it seems fine. Also when I test it, it works perfectly but two other people have tried to use it and had this problem. Could you help me?

From the screenshot you provided I see that your form is embedded. I checked your website, but I was not able to and your form. Please provide us the exact webpage URL where the form is embedded. Also, please tell us if this issue is only related to a specific device or browser.
