How can I find out who filled out a survey if they skipped the name field?

Since the only identifying information you are collecting on your form is the name and it's not set as a required field, there is not much you can do to find out who filled out your form except see their IP address. You can do this by selecting your form, clicking More>View Submissions>click on the Settings (the Gear button)>check the IP box. Like so:

IP address field will be displayed on all of the submissions for that form. You can add another identifying field to your form, such as the email or phone fields, and set them to Required by selecting the field, clicking on Properties (the Gear button) and setting Required to ON:
