NAIT School of Health and Life Sciences form creation support

Hi, 

I am looking to create a form for our simulation center but need background information on the specifics. I have created an outline of the form that I need in your free version but we need something more secure, most likely your silver membership with HIPPA security features.

Ulitimately, we need this form to populate into an excel spreadsheet on our sharepoint program for ease of information access and storage capabilities. 

In order for me to move forward with this I need to bring it to our IT department and I need more information. 

I would appreciate some help to make this process work.

Regarding adding the submissions to an Excel spreadsheet, you may manually download the data or send it automatically to Dropbox, the links below will help you doing so:

Unfortunately, we have do not have an integration with Sharepoint to send data.
