    Sorry to hear about this. Try changing the form's URL from

    https://form.jotformpro.com/form/21911266208954

    to

    https://secure.jotformpro.com/form/21911266208954

    and let us know if it helps. I've also cleared the cache from our end.

    Thanks, this worked. What concerns me is that I had to discover this
    problem was happening and was not informed that something happened from
    your side to cause this problem. Do you know what happened that this change
    was necessary?

    Our developers set the form SSL embed version to secure subdomain, so when a form is embedded to SSL , the location now is   https://secure

    You probably made a manual change to only  http rather than grabbing the code generated by the builder , which is   https://secure
