    Is there is a way to have multiple text area boxes listed under one item. Please see the URL that I've included for an example of what I'm looking for. Parts 2, 3, and 4 towards the bottom of the form are what I'm referring to.

    I think this should be possible. You can Inject CSS into your form to hide the labels via the field's id which you can get via the form's source code and then also you can additionally adjust the spacing so that it appears similar to that.
