I want to modify my form such that it has 4 text boxes when I enter a number, say 4.
For instance, when I ask how many subjects do you want to take in the first semester?
The student answers 4, there should be 4 text boxes below it.
Please help.

Another option would be to use dynamic widgets, which allows the user to add as many fields as they want to. You can add and try the following widgets:
