Is there a way that we could put a notes box on the form?

    I am part of a team that uses jotform, and I often have to forward the information to another party. Is there a way that we could put a notes box on the form for me to include a message to the next recipient when I forward the form to them?

    I think I need to know first how is that forwarding done. Is that the submission sent to your email address then you'll forward that to another recipient? Or, you forward the submission through the submissions page of the form? Or, you are utilizing the edit submission feature to trigger an email to another recipient?

    You could put a note if you are using the edit link feature. First, you need to add a hidden Long Text Entry field in the form. Second, add our Get Form Page URL widget in the form (you can put this at the top or bottom of the form).

    Third, configure a conditional logic to show the Long Text Entry field when the Get Form Page URL contains the text "edit"

    So, when you receive the submission, click on the edit URL. The get form page URL widget tracks the URL. It contains the text "edit", so the condition is met showing the long text entry field where you can add the note.
