How can I change the form alignment?

For some reason our application form became not aligned as initially created on its own...is there a fix for this by any chance? Please advise asap.

Are you referring to the labels alignment? 

Upon checking your form I can see this happens due to the shrink option turned on for those fields, I would recommend you to try turning the shrink option off and see if that helps. 

To find the mentioned option you may click on the field, the settings tab and go to the advanced tab:
