Pre-Populate from Form to PDF document

    I have set up an online form that I want to pre-populate a report in PDF. I can't seem to get the Field Details to repopulate the info in a text box. Is there any way to do this?

    In fact, you nearly completed this. There is one more step. You are just seeing the field names with PDF editor. When you download it, the values will be passed to PDF document.
    You can reach the city field in the address via {ministriesAddress:city}
    And {ministriesAddress:state} for state.
