I've taken a thorough look at your form's notification email settings - it's all good.
Also tested your form to see if it sends out the responses correctly and so far everything seems good.
One possible cause of this problem I could think of is that the user did not fill out the form and just clicked on Submit. To prevent this from happening again, set the fields to Required, you can do so by clicking on the field and then click on the Required button in the Form Builder toolbar.
