How to add a numerical value to each multiple choice answer?

    We are trying to automate a psychological wellbeing questionnaire. Every question has the same range of possible answers from Strongly Disagree to Strongly Agree. There are also several categories the questions fall into, but the questions are not laid out according to the categories. We would like to know if jotforms has the capability to:


    Add a numerical value to each multiple choice answer, by question

    Display the answers in assigned categories

    Send the completed questionairre only to the form creator (us) and not the person filling out the questionairre. (It is reviewed in person)

    Add a numerical value to each multiple choice answer, by question

    You may assign the calculation values to the multiple choice field so that they can be used in performing calculations with a calculation widget.

    Display the answers in assigned categories

    You may place the calculation widgets and the calculations on the form based on your requirement.

    Send the completed questionairre only to the form creator (us) and not the person filling out the questionairre. (It is reviewed in person)

    Yes. If you do not want to send an autoresponder to the person who filled it, you may delete the autoresponder from the form builder.
