For example, the second application was received but it also appeared to be from the first client. All the test applications have the name 'April Monnich' who was the first client. I just want to reset this-

Please try deleting the notification, and creating it again. You can make sure to put something unique in the email subject, such as the email, and name fields, so it will be different on each submission: https://www.jotform.com/help/544-How-to-Change-Email-Subject-or-Make-It-Dynamic

I have done a real submission for testing purposes in your form, and put my email address as the recipient, the email subject did not show April Monnich:
I have done a second test on your form, could you please check how it was received?

keenermandiReplied on May 20, 2019 at 3:43 PMIt won’t go to the recipient/applicant that way- because I have also tested it. It only comes to me that way- and the email you just sent me- the response- was from April- not from Jot Form, but from April

I have done another test in your form, and put my email address in E-mail field, and received the Autoresponder correctly, as being sent from JotForm.
Could you let us know what email provider you are using? Could you please try putting another email address as the recipient?
