I have tested your form using my iPhone 7 and I can't seem to replicate the issue at my end.

victoriadReplied on May 21, 2019 at 4:43 PMThank you for your response. I have done a lot of tweaking to see if I can fix it so maybe I was able to do that. I will continue working with ATT to see if I can get it to work correctly I am using an iPhone 6
