How can I get the formatting to work with embedded html code?

I have embedded the code on three different sites using both the single page of code as well as the version with js and css files split out. It doesn't look close to the form that is embedded. I need to embed html because I am trying to use third party code track UTM codes. Any pointers?

I notice that your web site is built using WordPress. Please note that when the form is embedded using full source code embed method, there may be conflicts with the other scripts on the web page. So, we generally recommend to use iframe embed code to display the forms on the web pages. Since you want to use third party code track UTM codes, you may consider embedding the form using the source code on a separate HTML web page and then display the HTML web page on your WordPress web site.
