Issues with the PDF form and missing answers on the email

    The PDF shows 1/4 of a page.  I have edited the PDF, it looks great, but comes out like that.

    On my form, the last three questions are for locations. Answers do not show in the email.

    We apologize for the inconvenience caused. I reviewed the submission PDF document on your form but so far I could not see the issue anymore.

    I was able to see all the fields including the last 3 texts with links below the form.
