Is this possible possible with Jotform?

Hello, 

I am trying to create a form that when submitted the completed form comes to my inbox as an attached pdf that looks like the form itself, not just text in the body of an email with the questions and answers typed out. 

I use Squarespace for my website but their form builder does not offer this. 

Is this possible with JotForm? 

Yes, that is possible. However, please note that online form and PDF reports have different templates so you need to customize them to your preferred design. I checked the picture you showed us and it is achievable in Jotform.
